How is Hair Transplantation Performed?

During the transplantation process, hair follicles taken from the nape of the neck are transplanted to the targeted balding area. (If the patient does not have hair, transplantation can be performed if a suitable donor is available.)
Hair follicles are called grafts. Rarely, in some cases, there is not enough healthy hair in the nape of the neck or temple area. If the nape is not enough, it can be supported by the beard. If the beard is also sparse, it can be taken from the chest.

 The transplantation process takes 6 – 8 hours, depending on the amount of hair loss.
 If the balding area is large, several sessions may be required to complete the treatment.

Our partner combines sedation with local anaesthesia. Local anaesthesia is also applied to the sedated person.

Sedation: Specialist puts the patient to sleep for 15-20 minutes with the effect of the drug given intravenously. While the patient is asleep, all the areas to be treated become numb, and then they wake the patient up and start the procedure.

The procedures are mostly performed under local anaesthesia with sedation.
After hair transplantation, only the donor part is bandaged with antibiotic cream.
The hair transplanted part remains open for the first day.
The bandage is removed from the donor part the day after the transplantation.
On the third day, the head is washed. (If the patient works in a place like a factory and will go to work after the transplantation, it is best for the patient to work with a special cap we will give.)

People with needle phobia usually request sedation.

 The patient is discharged 1-2 hours after the end of the process.

Although it is very rare, patients might experience a little bit of pain after the surgery. In such cases, pain can be controlled with painkillers prescribed by our doctor.

Is Hair Transplant Permanent?

One of the biggest questions in the minds of patients who want to perform hair transplantation is whether the transplantation is permanent or not.

To give a short answer, your transplanted hair is permanent. The area where your hair is taken is called the donor area. This area is also called a “reliable or strong area.”.

The donor area is an area covering the nape and upper part of the ear. The hair in this donor area is insensitive to the DHT hormone that causes hair loss. The hair follicles in the donor area are of very high quality and healthy.

In general, the donor area is genetically programmed against hair loss.
For example, even when someone who has experienced hair loss from a young age reaches their 70s, you will see that the hair in the donor area is still in place.

Since the follicles used in hair transplantation are taken from the donor area, which is not affected by hair loss, they have the same genetic codes and characteristics as the donor area. Therefore, as long as the hair in your donor area continues to grow, the hair in the transplanted areas will also continue to grow.

Sometimes hair loss problems affecting the donor area may also be encountered. In this case, the hair in the nape area will also fall out. However, the hair usually grows back when this problem is solved.

Approximately 2-4 weeks after hair transplantation, 95% of hair transplant recipients experience hair loss. This is called shock shedding. Shock shedding: the grafts taken from the donor part remain in the special solution we keep for 3–4 hours. Naturally, they do not die, but since the nutrition in the existing place stops, when the grafts are placed in the transplanted area, they leave the hair strands on them for an average of 2-4 weeks. Then, after 3–4 months, they start to grow new hair strands, and a temporary hair loss may occur after about 2–4 weeks.

This is a normal reaction expected from the body. It should not be feared, but the course should be observed. This loss is a trauma caused by the body’s reaction to hair transplantation. Almost all cases are temporary. This early hair loss process is followed by another process that continues with normal hair growth, and everything normalises.

The permanence of your hair transplantation depends on some other factors. The following points should be considered for permanent success:.

How Long Does the Procedure Take?

The duration of hair transplantation varies depending on the size of the area to be transplanted, the number of hair follicles, and the method used. In the FUE method, hair transplantation duration is usually between 6 and 8 hours.
In the DHI method, hair transplantation duration is usually between 4-6 hours.
The recovery time after hair transplantation is approximately 2–3 weeks.

Elements that are affecting the duration of hair transplantation are as follows:

  • The size of the area to be transplanted is: The larger the area to be transplanted, the longer the hair transplantation time.
  • Number of hair follicles: The more hair follicles to be transplanted, the longer the duration of hair transplantation.
  • Technique: The FUE method takes longer than the DHI method.
Is Hair Transplant Painful?

Since hair transplantation is performed under local anaesthesia, almost no pain is felt during the procedure. However, mild pain may be felt for a day or so after the transplantation, especially when the effect of local anaesthesia wears off.

The pain you will feel will be kept to a minimum with the painkillers given by our doctor. In addition, redness, pain, and discomfort in the transplanted area may be felt for a few days after the procedure. It should be noted that most of our patients did not need painkillers in the post-transplantation process.

When the procedure is started, you may feel some pain during the local anaesthetic injections. The level of pain you will feel is very close to the pain of the needles made to prevent you from feeling pain when you go to the dentist.

You may feel tension in your scalp during the procedure. This feeling of tension is felt more in transplants performed with the FUT technique.

Since the pain threshold varies from patient to patient, some people describe this feeling of tension as “mild pain,”  while many of our patients say that they feel almost no discomfort.

The aim of hair transplantation is to improve the aesthetic appearance of the patient. The level of satisfaction with the procedure is quite high, and this causes many patients to ignore temporary side effects such as rash and ache that may occur.

Are there any risks or complications?

Hair transplantation performed by a specialist doctor or a health care specialist has no harm.

Although this procedure takes an average of 4-6 hours, it is not likely to cause any harm to the patient.

Hair loss also affects women, and they may want to know if the procedure is suitable for them.
Hair transplantation is not suitable for people with serious health problems such as heart disease, diabetes, liver failure, or kidney failure. Hair transplantation is not recommended for people with high blood pressure and diabetes.

Will I Need Additional Treatments After a Hair Transplant?

For the first 10 days, you must wash your hair every day with the recommended shampoo and lotion. Do not take off the headband while washing; you can let it down on your neck. Try not to get sweaty, and protect your scalp from heat and sunlight. You should also restrain yourself from doing any sexual activity. 15 days after the surgery, use the fingertips for 10 minutes once a day to massage the transplanted area with a circular motion.

How soon will I see the results?

It takes approximately 1 year to achieve the desired result in hair transplantation and to have a natural appearance. 

During this period, it is important to check your hair and head regularly and follow your doctor’s recommendations.

How many grafts do I need?

The maximum number of grafts in hair transplantation should be 4,000. It is not recommended to transplant more than 4,000 grafts of hair in one session.

Am I a good candidate for hair transplantation?

There are two methods known as Sapphire FUE hair transplantation and DHI hair transplantation (the unshaven transplantation technique).

People who want to have hair transplantation should have either complete hair loss in the area where they want to have hair transplantation or, in the case of thinning, the hair loss should have stopped completely.

Sleeping After a Hair Transplant 

Although hair transplantation is a smooth procedure, it is a surgical operation performed in a surgical environment. Therefore, the area of the operation can be quite sensitive, especially in the first two weeks after the application. At this point, many hair transplant patients may have concerns about how they should sleep in the first weeks and wonder which of the sleeping positions will be healthier after hair transplantation.

First of all, we should say that all patients after hair transplantation, regardless of which technique they choose, have some swelling and oedema in the first week. This is your body’s natural reaction to the procedure. Although this oedema and swelling caused by our body’s defence system are intense in the first week, they disappear spontaneously over time. Although oedema and swelling will definitely occur, small precautions you can take can minimise the oedema and swelling that will occur.

One way to minimise oedema and swelling after hair transplantation is to ice compress, but another way is to sleep in a position that keeps your head up for the first week after the operation. When you sleep in a semi-sitting position with your head elevated, the oedema and swelling in your face and forehead area will decrease, and you will protect the recipient and donor areas, especially the transplantation area, from possible compression and trauma.

Sleeping in the fetal position after hair transplantation may seem like a good idea because it will protect your nape area, but since your head will be tilted, sleeping in this position both increases swelling and oedema on your face and causes joint pain.

Another unfavourable position to be preferred is lying face down. Only one part of our body is affected after hair transplantation. Every neighbouring tissue connected to each other is affected by hair transplantation. Therefore, it is not enough to protect only the back of your head. At this point, it is necessary to act by thinking holistically. When you lie face down, you may be protecting the bandaged donor area on the back of your head, but you increase the oedema on your face and forehead due to the position.

The position that will make you comfortable after hair transplantation is to sleep in a semi-sitting position for the first week and not touch the pillow to the area exposed to the hair transplant procedure. At the end of the first week, you can sleep in a more reclining position with the guidance of your doctor.

What is the success rate of the procedure?

While the success rate after hair transplantation in men is usually between 95 and 98%, this rate is usually between 85 and 95% in women. The reason for these differences is that hair loss in women is based on a more complex cause, and hair transplantation requires a more difficult technique in women.

What should be done if hair loss continues?

If the person’s hair loss continues, it is requested to postpone the hair transplant operation until the hair loss stops or to benefit from anti-hair loss treatment methods. Generally, hair loss is genetic. Therefore, we recommend hair transplantation operations to individuals whose hair loss has completely stopped before the age of 25.

People who do not have any health problems can plan the hair transplantation procedure in a full-fledged hospital environment after a preliminary examination by expert teams in the field and if no problems are encountered as a result of the examination.

Does getting sweaty damage hair transplantation?

Hair transplantation can be performed safely in all seasons, and you can get successful and satisfactory results at the end of the day. However, if hair transplantation applications are to be performed in hot weather, there are some rules to be considered. For example, the transplantation area should not be exposed to direct sun for the first 1 week and 10 days. Since hair transplantation applications affect the skin surface and the scalp will become sensitive for a while after the procedure, direct sun exposure after hair transplantation may damage your hair transplantation process.

Another issue to be taken into consideration is sweating. However, this includes not only the sweating and heat caused by the weather in the summer months but also the sweating you will experience due to sports, sexual activities, or various exercises.

Sweating is a condition that affects the scalp. For this reason, you should try not to sweat, especially in the first 10 days after hair transplantation. During this period, you should not be directly exposed to the sun, you should avoid exercises that will tyre you and cause you to exert effort, and you should practice sexual abstinence.

At the end of the 10-day period, your scalp will start to heal, and you can end your abstinence and start activities, provided that you do not tyre yourself too much. In this case, you can consider the answer to the question of whether sweating will harm hair transplantation as yes for the first period of hair transplantation. In the following times, after your scalp starts to heal and the transplanted grafts are attached, there is no harm in sweating. You can continue your social life as usual.

You may have thought about having a hair transplant, but you may be postponing it for reasons such as its price and the fact that it is a treatment that requires a surgical procedure.

The most successful treatment for hair loss today is hair transplantation. Although it is a relatively long operation (6–9 hours), the success rate of the procedure is quite high when performed by a specialist surgeon and an experienced team. In this procedure, genetically resistant to hair loss, hair follicles on the nape and sides of the head are collected by a surgical procedure and transplanted to the areas where hair is sparse.

The hair follicles on the back and sides of the scalp are much more robust than the other parts and are genetically programmed not to fall out until the end of life. Therefore, when transplanted to other areas of the scalp, they retain this characteristic and continue to grow healthily where they are transferred.

There are lotions and medicines you can use to block the effects of hormones that cause hair loss, but these are treatments that require lifelong use. When you stop using them, hair loss will probably continue. Currently, there is no other long-term alternative that can solve hair loss with a single procedure, such as hair transplantation. The only treatment that can come close to this is a solution called ‘hair simulation’.

What is hair simulation?

Hair simulation is an innovative treatment also known as hair tattooing or hair shading. In this method, which does not require a surgical procedure, the appearance of short hair on the head is obtained by using various tattoo techniques, so it does not provide new hair growth on your head; when viewed closely, it is only an image.

Hair simulation specialists, as in normal tattooing, use very fine-tipped needles specially produced for this treatment, using natural and herbal dyes to create the impression of very short-cut hair on the scalp and achieve an image integrated with the natural hair of the person.

On a completely or partially bald scalp, it can be used to create a more forward hairline, to cover open areas on the crown and sides, or even to complete sideburns. It can also be used to cover scars, sun spots, or birthmarks, as well as to enhance the appearance of the scalp after hair transplantation. Women who prefer long hair can choose the hair simulation process to cover the places where the hair becomes sparse and the scalp is visible.

Micropigmentation of hair can be used on all skin types and colours. Experts use a dye that perfectly matches the natural hair colour and usually use three or four different shades to achieve as natural an image as possible. Our hair is made up of hairs of multiple colours and shades, and a good micropigmentation process reflects this exactly.

Like hair transplantation, this is a long-lasting procedure, but unlike hair transplantation, it is often completed in a few sessions. It is similar to the permanent makeup technique used in licensed beauty salons or aesthetic centres to give the appearance of fuller eyebrows.

Clothes That Cover Your Hair 

You can choose to accept hair loss and live with it, or you can even love the way you look. You can also wear hats, headscarves, or wigs to hide hair loss. There are also coloured sprays that you can use to cover the thinning areas. However, all these techniques have disadvantages. For example, you have to reuse the coloured sprays after each hair wash, and they can stain places where they come into contact with your hair, such as beds, sofas, pillows, and clothes. 
This is why many people avoid the above-mentioned methods or give up after a while. 
Our hair is often an important part of our identity, and we know that hair loss is a sad and sometimes even embarrassing reality.

Lotions and Sprays for Hair

Another alternative to hair transplantation are solutions and sprays that you apply directly to the hair, such as Rogaine or Regaine (also known as minoxidil, the active ingredient). These need to be applied to the scalp twice a day and usually need to be used for at least four months to get noticeable results.

They can help revitalise hair and prevent hair loss, but you need to keep using them for life. If you stop using it, you will probably continue to lose your hair.

Minoxidil is only suitable for men aged 18–49, and people with blood pressure problems should not use it. It can also cause a decrease in libido and impotence.

There is a different minoxidil treatment for hair loss in women. This treatment can be used in women aged 18 to 65 who are not pregnant or breastfeeding.

Pills for Daily Use

There are also pills you can take to prevent hair loss. These suppress the action of hormones that can cause hair loss. However, these are also lifelong treatments; within a few months of stopping the medication, new hair growth will stop, and you will notice that your hair is becoming sparse again.

In rare cases, some of these treatments can cause impotence and a low sperm count in men. They are not suitable for use by women with hair loss. Most of these medicines start to show their first effects at least 3 months after you start taking them.

Laser

There are various laser and LED light devices on the market that can be used at home to help stimulate hair growth. Most of them need to be applied several times a week and come in different models, either applied to the hair like a comb or as devices that you wear on your head. They often use low-level light therapy (LLLT) to help stimulate ageing hair follicle cells and help regrow healthy hair. Although there are some that are designed for home use, we recommend that you have this treatment at a licensed health centre under the supervision of a doctor. 
We do not yet have enough scientific evidence on the exact mechanism by which these devices treat hair loss or whether they actually work. 
There are also various vitamin lotions and shampoos that promote new and healthy hair growth when used in combination with various hair loss treatments.
